Description OBJECTIVE: This policy note provides observations from a pilot that tested integration of social accountability mechanisms in healthcare delivery in Kenya between 2011 and 2013.

The Constitution of Kenya (2010) provides that most functions of the State are decentralised in a devolution process. Health is one of the key sectors whose functions have been devolved. The 47 county governments elected in March 2013 are responsible for managing all aspects of service delivery while the central government is responsible for regulation through policy formulation and monitoring. These provisions in the Constitution were formulated with the goal of boosting efficiency and accountability.

The devolved health system is four tiered: community health services, primary care services, county referral services and national referral services. All but national referral services are managed at county level.
